Visit the school and get an admission form from the school's account office.
According to your child's class go through an admission test if your child passes it then according to a fee structure you have to submit a fee to the relevant Bank.
Documents required are:

Student picture (passport size).
Father's / Guardian's I.D Card copy.
Student's health certificate & B-Form.
Admission form (filled).
